School Nursing Targeted Intervention Service

School Nursing Targeted Intervention Service offers a service to all children and young people age 5-19, and up to 25 years for SEND young people and their families. 

Providing targeted intervention to support, physical and emotional well being in order for a child or young person to attend their education and achieve to the best of their ability.

Working in partnership with schools and other professionals, we offer support to children and young people with SEND in various ways.  This may be through signposting families to other services, health screening such as hearing tests, care planning and training teachers to support your child's medical needs, providing a report to support your child's application for education health care plan and where necessary completing an assessment for continuing care application.

  • speech, language and communication difficulties
  • autistic spectrum condition
  • ADHD/ADD
  • learning difficulties
  • complex medical needs
  • long term conditions
